Professional Development Manager / Care Coordinator

Sean Hammond has worked as a Care Coordinator in the infectious disease and HIV field for the past thirteen years. He has spent most of his professional career providing Ryan White Case Management services, but also specialized in Medicaid PAC Waiver Social Worker/Medical Case Management for his patients. 

Sean is one of our newest team members here at Hope where he is now serves as a Senior Care Coordinator. He feels a personal connection, and responsibility to all those he has served over the years, as he too has been HIV+ since 1999. Sean’s commitment to the well-being of his patients is something that leaves a lasting impression on many of them. Many of which have followed him, throughout his career here in Jacksonville, so they can continue receiving the great continuity of care and support he has provided.  

Sean earned his Bachelor of Fine Arts (BFA) from Jacksonville University, served as Chapter Secretary for the Association of Nurses in Aids Care (ANAC) for ten years, and is currently working on obtaining his Master’s in Healthcare Administration (MHA) from the University of North Florida (UNF). 

Sean has been married for the past year and a half to his partner of nine years. They both love to travel to far-off destinations to enjoy scuba diving, soaking up local culture, and enjoying cuisine from around the world.
